Schedule an appointment
Use our online calendar to book an appointment with any of our providers. New patients are welcome. If you have questions before booking, we are happy to chat by phone.
New patients
Choose your provider, select New Patient, select your desired appointment day & time, and then fill in the required fields to set up your patient portal.
All patients are required to keep a valid debit/credit card on file for copays, dispensary purchases, and no-show fees.
Medicaid patients: Even though you will never receive a bill from us, EFM has too many active patients to keep track of each patient’s insurance plan. By requiring all patients to have a card on file, we are able to streamline our systems so that we have more time available to assist you. Thank you for your understanding & cooperation.
The front office will review your appointment request, call you if any changes need to be made, and then approve your appointment. Once approved, fill out & submit your intake questionnaire(s). Note: This may not work on all mobile devices, so it’s best to use a computer for this step.
Intake form(s) must be completed no later than 2 business days before your appointment or your appointment will be cancelled. You will receive a phone call if we are cancelling your appointment due to an intake form issue to work to resolve that before losing your spot.
General Guidelines
New patients must schedule a New Patient appointment as their first visit.
Last-minute online bookings are limited. If you need to be seen urgently but can’t find a spot on the online scheduler, please call our office at 206-328-7929 to be fit in.
Some appointments, such as injections and blood draws, are not available to book online. If you don’t see your appointment type on the list, please call the office to schedule.
We have a 1-business-day cancellation policy. This means if your appointment is booked at 10 am on a Monday, you must call us (voicemail or ChARM message is fine, too) no later than 10 am the preceding Friday to cancel or reschedule. If you give us less than 1 business days’ notice, your cancelled appointment will be counted as a no-show.
